Prime Healtcare advised that employee Susan J. Horvat's history shows Enrollment Terminated and Enrollment added by "EDI Test." Screenshot attached: 
Prime Healthcare is asking that WorkTerra run reporting showing any entries made by "EDI Test" that changed plan terminations from 2017 to a December 31, 2016 date. This needs to be done for all Prime Healthcare facilities:
