          ALL - the forms are wrong. Line 16 is not populating for employees even though they have been enrolled in Medical for all 12 months in 2015. Examples of employees - Tressa Abraham, Jourdene Rosella Aguon, Jennifer Aldo, Dales Anderson, Dawn Beal, Claudette Chavez, etc. Ton of issues; cannot send to the client as is.

          Customization :

          This client 1-095 c report the UHC provider medical plans are not selected so thie attached employees are not getting the Medical offers.

          *Solution *:

          We have changes the customization and selected All the medical plas now these employees are getting the 1E and 2C codes.
