We need to correct the below EE's 1095 data. It appears we are pulling the lowest cost for Cobra plans on line 15 for EE # 1/ 2. #3 needs data correct on line 14 as she had coverage through December.
1. David Pope
2. Jean Homiski - Cobra rate showing for lowest plan cost.
3. Christian Lappin
EE was laid off at the end of December as indicated in my letter (attached) on the 14th of December that my position was being eliminated at the end of the year.

As requested by Windstream Company,we are showing class wise costs for employees.
Please refer attached excel which was provided to us. Windstream Cost Matrics and EINs.xls
We are using column "Provided Monthly Costs" from Costs tab for displaying costs to employees.
David Pope and Jean Homiski : These employees are getting costs as per their class.

Termination Date:12/18/2015
Codes displayed are as follows:
1H-1A: Jan-Sept: Employee not employed.
1H-2D: Oct : EE was hired in Oct,so he is in Limited Non Assessment period in the month of hire.
1E-2F: Nov : EE had offer which was affordable as per W2 safe harbor
1H-2B: Dec : EE was termed in Dec.Offer ended due to termination.
