Explanation to the common issues:
A. For 2B code
For 2B following conditions need to be satisfied:
1. Employee not a full-time employee. Enter code 2B if the employee is not a full-time employee for the month and did not enroll in minimum essential coverage, if offered for the month.
2. Enter code 2B also if the employee is a full-time employee for the month and whose offer of coverage (or coverage if the employee was enrolled) ended before the last day of the month solely because the employee terminated employment during the month (so that the offer of coverage or coverage would have continued if the employee had not terminated employment during the month)
In the issues mentioned are mostly for part time employees to display 2B code.But such employees do not fall under 2B Condition(1) as they do not have offers.So 2B is not displayed.
B.PT to FT
Employee status is changed from PT to FT,But this change does not change division.So there is no EIN change.
If there is EIN change then,1H-2D will be displayed if applicable else if employee is enrolled,1A-2C will be displayed.
C. COBRA Reporting
For COBRA Employees
An offer of COBRA continuation coverage that is made to a former employee upon termination of employment should not be reported as an offer of coverage on line 14. For a terminated employee, code 1H (No offer of coverage) should be entered for any month for which the offer of COBRA continuation coverage applies.
Do not enter code 2C in line 16 for any month in which a terminated employee is enrolled in COBRA continuation coverage (enter code 2A).
COBRA COntinuation Coverage is not considered in Part 3.
D. For Retiree Employees
The reporting would be similar to any termination/COBRA coverage situation.During the year an employee retires, there is a Form 1095-C reporting requirement.For the months following retirement, lines 14 and 16 would be coded with 1H/2A, respectively.Part III would be completed only if the plan is a self-insured plan.There would be no Form 1095-C reporting requirement for the year following the year of retirement for a fully-insured plan.With a self-insured plan, an employer could decide to report the retiree coverage using either Form 1095-B or Form 1095-C.If Form 1095-C is used, the employer would following the COBRA reporting rules for self-insured plans.
E. For 1G code
"Use this code when offering self-insured coverage to an employee who was not a full-time employee for any month during the calendar year. This may include one or more months in which the individual was not an employee. Enter code 1G in the “All 12 months” box and do not complete the monthly boxes."
So if 1G appears even for a single month,then all 12 months will be reflected with 1G.
Also some of the codes requested, leads to invalid code combination,which may not be accepted by IRS while submission like 1A-2A

In much earlier mail, client had told to mark line 14 as 1H and line 16 as blank.
This is the content of mail:
Puerto Rico Employee and Jones Act/Claims Employees:
These employees are not eligible for coverage due to 1) living out of the country and there is not a network of doctor’s in PR or 2) they were off work under the Jones Act (claims) and did not meet their eligibility requirements as of their eligibility date so benefits were “waived” until they are released from claims.
Line 14 code for all months should be 1H (includes month where the employee was not employed and months where the employee was employed).
Line 16 should be blank for months the employee was employed by Marquette. There is not a code that matches the description as to why the employee is not eligible for coverage. Per Tim, this should be left blank.
