- add check boxes | Drop downs | Radio buttons in a form and use these fields in additional eligibility in eligibility rules
- also use standard macros like enrolled in
- go on summary for this rule and view it's description
Description for this rule shows the filed ids instead of showing their label as they are shown in actual additional eligibility

Please refer following 3C's:
Concern: WT -> Generic Eligibility Rule Summary -> Showing inappropriate filed ids instead of showing just their labels in rule descriptions
Cause : In this sp to fetch additional field we were using DestinationTableColumnName .
So because of mismatch in DestinationTableColumnName and display label there was inconstiancey in the display.
Correction : No we are using DispalyLabel to fetch additional field details.
