Description for this rule shows the filed ids instead of showing their label as they are shown in actual additional eligibility

Tested this on Stage - 31st Aug 2016 - Caiso for HSPL
Working Well
Tested with additional fields like drop down, check box, radio controls and macros like enrolled in etc.. with demographics as well as on user defined forms
respective rule's summary shows appropriate field's name instead of showing the ID

Please refer following 3C's:
Concern: WT -> Generic Eligibility Rule Summary -> Showing inappropriate filed ids instead of showing just their labels in rule descriptions
Cause : In this sp to fetch additional field we were using DestinationTableColumnName .
So because of mismatch in DestinationTableColumnName and display label there was inconstiancey in the display.
Correction : No we are using DispalyLabel to fetch additional field details.
