RCF- JE DUNN - EE enrolled in Medical flexible spending, has an annual amount but not a per pay period amount

JE DUNN employee Brian McElroy enrolled in the Medical Flexible Spending. The enrollment and the confirmation statement shows the Annual amount but it shows 0.00 on the per pay period amount. Why is it showing 0.00 for the per pay period amount?

In the past this had also happened to EE Sarah Meeks but for Sarah it was the Dependent Care Spending Account. To fix this EE I had to do an Admin event, waive coverage and put it back in order to fix it.

Please research why EE Brian McElroy enrolled in the Medical Flexible Spending, has an annual amount but has 0.00 for the per pay period amount. I had opened a CT ticket on this but CT hasn't been able to resolve yet.
