DEM:
· The header doesn’t appear to have correctly delimited all the fields. For example it shows “Minimum,Coverage,Amount” rather than indicating that as a single field.
· None of the EINs we received were valid
· We received a lot of fields with whitespace rather than being completely empty
· The payroll frequencies don’t match the codes indicated in the file specifications
· Employment status is missing for everyone
· Opt-out flag is an invalid value. Please see the codes in the file specifications
· Termination flag is an invalid value. Please see the codes in the file specifications
DEP
· We shouldn’t receive names in both field five and fields 8-10. We should receive one or the other and we strongly prefer receiving the names in parts within fields 8-10
· We are receiving whitespace in fields that should be empty
· We’re missing a lot of DEP SSNs. This is allowed but will ultimately require additional employer solicitation to the get the SSNs
· We aren’t receiving valid EINs

Hi Van,
Thanks you for the file. WE’ve actually already received two sets of test files and I believe the issues with those files were identical to this one:
- We are receiving an SSN in the ‘old ssn’ field for every person. This field is meant to be used when it’s necessary communicate that a person’s SSN is changing. You pass the old SSN we’ve already received in the ‘old ssn’ field and then the correct, new SSN in the regular “SSN” field.
- We are receiving a lot of plan IDs that we don’t have in our system. We’ll need you to send over a description of all these plan IDs that includes the following information for each plan ID:
o A description for understanding what plan it is.
o The dates the plan is valid for, start and end (usually the plan year)
o The minimum default cost for the plan (should we not get a minimum cost for an individual on the DEM file who is in the plan)
o The month the plan validity starts
o Whether or not the plan covers employee, spouses and dependents.
If so, is the offer spouses conditional?
o Self-insured or fully-insured?
o Does it meet minimum essential coverage (MEC)
o Does it meet minimum value (MV)
o Is it a multi-employer plan
o Should we set the plan as the default for people if we don’t receive plan ID for someone - There are a number of rows that we didn’t receive a minimum plan cost
Regarding the SFTP connection, I put in a request to whitelist the additional IP addresses. There was also a request to send new credentials that were sent on the 31st. Are you using those credentials to send the file?
Lastly, I hope it is understood that we need to receive a DEM file for each month going back to 11/1/2014.

Also, the data in our systems only go back to 1/1/2016, so we only need information back to 1/1/2016. If you can also provide a list of all Plan IDs as Tango requested, which should've came from the client, they will need that.

We found some dough full employee while testing , currently we are doing analysis of data.
Plan ID is shared by client in file Name- Plan IDs 11.11.2016.xlsx is match with system Plan ID (Local Environment and Production ).
We have selected shared plan name file Name- Plan IDs 11.11.2016.xlsx while exporting data for Wheaton Franciscan Healthcare on Production.

I’m not certain this is a full file, unless I am confused. This file appears to only contain Wheaton employees enrolled in the medical plans, not all Wheaton employees that are eligible for the plans. I think that is the reason that you see a benefit effective date for everyone.
The file should include all employees, not only those enrolled in a medical plan.
The file contains 9,427 records, the Workterra database shows a total of 17,071 total employees.
I think the file is missing those that are not enrolled in the medical plan.
Also, the Employment status field indicates FT for all employees. This is not correct as PT employees are also benefit eligible.
Also, I think the Minimum Coverage Amount needs to be $71 for all enrolled employees except those in the FMI Kaiser CO plan and those should be $207 per month.
I am also concerned with records that are missing, for example Debra Blonski, showing in Workterra with UMR Basic coverage from 1/1/2016 – 8/31/2016 (Under and ACA plan). She was subsequently rehired with coverage starting on 9/1/2016. While this is a bit of an unusual case, she is not showing on the Demographic file provided earlier today.
Same with someone like Lisa Anderson, who was rehired on 8/8/2016, with medical benefits effective 10/1/2016, she is not represented on the report either.
I also question the record of Heidi Anderson as she is showing a coverage Eligibility date of 1/1/2016 with a Benefits Effective date of 1/21/2016. I show her benefits actually beginning 1/1/2016 not 1/21/2016. These need to be looked at and a new file sent over.

I apologize, I spoke with the carrier and they stated that COBRA and COBRA-Active members do need to be on the file. Can you add them back? This is not typical of most ACA files that usually only contain active members, but because these plans are "self-insured" plans, not fully insured plans, then they are required to be on the file.
Do you know who's working on the dependent file? It is separate from the demographic file. Dependents need to be reported if they are on "self-insured" medical plans.

This associate has a Medical Plan enrollment effective 8/1/2016 - 12/31/2016
The dependent file was also been sent to Tango and feedback was provided to Workterra. The original files including the dependent file were created by Workterra on 9/16 with a subsequent file on 9/23. That file needs to be provided along with the Demographic file to Tango as soon as possible. I have also attached the Analysis document from Workterra dated 9/23 on both files.
These files were to be in production by October 28th in order to allow 12 months’ worth of files to be loaded, evaluated and time for corrections to be made prior to Federal Reporting requirements.
A service estimate was signed on 9/2/2016 for Workterra to meet these requirements. Please let me know if you have questions on how the Workterra site was set up for Wheaton or contact Owen Pickford at Tango as he is the expert on the Tango file specs. We need these files to be corrected, finalized and sent to Tango as soon as possible.

Thanks for the new file. I reviewed it and it is basically good. The only things I found concerning were mentioned on our call already:
- You are working on including all active employees on the file
- We need to get data for people who are on self-insured plans, including COBRA participants. Typically COBRA people are passed in a separate file. I’ve attached our most recent data specifications here again so you can see how we manage that typically. It’s basically a DEM file but with different fields that are mandatory versus optional.

BSG is very upset and have escalated the issues to management in regards to these files not being live yet. They want an estimated date of completion for when the next test files will be available and want these files to be done as soon as possible.
DEM: EBS to work on including all active employees on the file. EBS needs to have data for people who are on self-insured plans.
COBRA: Typically COBRA people are passed in a separate file. It’s basically a DEM file but with different fields that are mandatory versus optional. See "Tango ACA Concierge File Specifications 2-1-6.pdf"

[Tango Comments] - it appears like the file is missing reserved field, in the position number 5.
[Harbinger] - We have export data as per first requirement sheet shared with us. Tango File Specs.pdf . As per this specs -position 5 is - Dependent Full Name. I think client compared this DEP file with new specification which you had shared - Tango ACA Concierge File Specifications 2-1-6.pdf
[Tango Comments] - The most recent DEP file we received was missing field 5 entirely. The dependent name was split up into parts but in positions 7, 8 and 9 instead of 8, 9 and 10. In an older version of our specifications, field five allows clients to send the entire dependent name in a single field. We strongly encourage against using this field. However, the field must still be present but it should be empty.
[Tango Comments] - It also appears like the full middle name is being passed instead of middle initial. If you can’t pass middle initial you could leave this field blank.
[Harbinger] - We set the length for middle Name on DEP template. Hence forth file will have only first digit of middle name.

1) The differences between V2.1.4 specifications and the V2.1.6 are on page 3 and 4 of the Tango ACA Concierge File Specifications 2-1-6.pdf specs.
2) Please set "RC" for Employment Status on the COBRA DEM file.
3) Retirees do not need to be included on any files. There's only 1 retiree that's being processed manually by the client.

Yes, we can do that. A few things to keep in mind:
• Make sure that employees are only on one file or the other and remain consistently that way across the Demographic Months.
• If an employee transitions from one file to the other (such as not enrolled to enrolled) the month they need to appear enrolled they should only appear in that file and not the other.
• Use the same naming convention for the filenames, but you can adjust the timestamp to keep track of the difference.
o As a reminder, files shall be named using the three letter file type code (DEM, BEN, RET, PAY, DEP, or LOA), followed by the reporting entity's EIN (Employer Tax ID), followed by a timestamp (YYYYMMDDHHMMSS). The timestamp should represent the effective date of the information in the file, often the time the file was generated. For example, a Demographic file for an EIN 99-9999998 containing data accurate as of July 31st, 2015 at 12:00 PM would have a filename of DEM99999999820150731120000.
o If you have two files representing July 31st 2015, you could have the enroll file be one timestamp (like 12:00 PM) and the not enrolled be another (like 1:00 PM).

MEC Risk:
Large number of employees lost benefits and then have a gap before they are formally terminated. Causing a MEC issue and needs to be reviewed. We believe these are employees who are going on leave and a Benefits Eligibility Termination Date is passed due to non-payment, but it should be excluded since the offer is still valid; instead an opt-out date should be passed. Kim/Van – is there any programmatic way to fix this? We will not be able to edit the files ongoing and interpret. Ideally, someone who loses benefits due to non-payment would pass an Opt Out Date and only when they terminate would you pass a Benefits Eligibility Termination Date. Also, need to make sure for actives who have a qualified life event and drop coverage but remain eligible are working correctly.
Code Coverage Report Blockers:
DEP file not formatted properly
Please refer to file specs: Missing Column 5 (Reserved) and Columns 9 and 10 are reversed. (Should be 9: Middle, 10: Last Name) Van – Communicating to programmers to change going forward. Due to timelines Tango will programmatically adjust existing file but December’s DEP file needs to come this way.
*ii. DEP had dependents listed for an employee who was not on the DEM file
1. Kim – Will review file from Scott to determine who they are, but Van may need to review as well.*
Also, please provide a test file that includes that are included in the "Employee_List_Who_Neither_Enrolled_Nor_Waived.xls" file in the DEM/DEP active format. Tango will need to load these records separately from the rest of the population. However, they need everyone to be included in the full population files on a monthly basis. Please include opt out date and if they terminated employment then please pass termination date.

[Tango] – Rachel Goelz should not show up as a terminated record at all, because her benefits have been active between 1/1/2016 – 12/31/2016 for ACA reporting. She did add a dependent on 1/12/2016, but how she showed up on the DEM file is inaccurate.
[Tango] – Karen Hanley – she was listed on the 12/8/2016 test file without a Benefits Eligibility Date. Please use a default date of 1/1/2016 for all employees and dependents or the actual Benefits Eligibility Date based on company new hire or effective date rules.
Also, Tango needs a DEM/DEP file with December data because it is currently missing.
Tango also stated that there are some Active records that are showing up as Terminated, when they shouldn’t be showing up as Terminated since they Opted out of benefits. They need to be in Opt Out status.

Feedback on the test files
Scott at Tango: Kim and I have been discussing a possible issue with the COBRA files that we need to discuss ASAP. We believe that the COBRA file may contain terminated employees but information on their benefits while they were active, rather than their COBRA benefits.
Kim at BSG: Matthew Anderson is an example. He is showing enrolled in the ACA plan, below is some information I sent to Scott earlier today. Please call me if you would like to discuss further.
The ACA plans that were identified in Workterra are for enrollment in a medical plan that occurred prior to the transition in enrollment vendors. We used the ACA indicator to identify these terminated employees as having coverage within the 2016 calendar year while they were active employees. We needed to capture employees in a terminated status to load them into the system but did not want to give them an active status during the transition to the new enrollment site. This was the way of capturing that data. For Example, Matthew Anderson, SSN xxx-xx-1654 came across on the COBRA file that Van provided to me with enrollment in the ACA Medical Basic Plan from 1/1/2016 – 4/30/2016. He is a terminated employee with a termination date of 4/29/2016. He carried Family medical coverage while he was active and this ACA plan represents that Active coverage.
I think the confusion lies with the ACA plans and the COBRA plans. ACA plans were used for employees that had medical coverage that was elected while they were an active employee of Wheaton during 2016. When they terminated employment their medical election terminated.
Since we did not transition to Workterra until September 2016, we needed a way to capture the historical medical enrollment on terminated employees without sending any data on EDIs so different ACA only plans were created to capture that historical enrollment information.
The COBRA plans were set up as well, but our COBRA area did not maintain elections within the COBRA site but rather within our Travis COBRA site. We are working now to get that COBRA enrollment information captured so that COBRA enrollment reports can be generated out of Workterra for COBRA.
Scott did indicate that we could focus on getting the Active and ACA plan enrollment correct and then work on the COBRA and print forms for COBRA participants by March. I think that will be our best plan of action.
Let me know if you have questions and I apologize for the late recognition of this issue. Part of the problem of never really getting the data or examples of the data to review and having Workterra and the vendor working together when you don’t have a full understanding of the data.

Scott from Tango: Matthew Anderson – Active and enrolled in medical plan effective 1/1/2016 – 4/30/2016. He didn’t show up as terminated until Sept. (retro term) – he should’ve shown up on the COBRA file on the May file and should not have shown up on the January, February, March or April RET (COBRA/Retiree) files until May.
His employment was terminated effective 4/29/2016, but the “Termination Effective Date” field shows 9/9/2016, most likely because his actual employment termination date of 4/29/2016 was not provided to Workterra. There are 966 records with this “Termination Effective Date” on the Active DEM files. Please refer to Active DEM 0731 file as reference.

Kim from BSG: 9/5/2016 – This is the date Wheaton went live with WT. Most likely, the date the data was loaded on 9/9/2016 was used as the employment term date for those that were terminated. Please refer to Amit Thorve if the payroll files he used to import the data into WT in fact had missing Employment Termination Dates. Kim from BSG will try to provide a list of Employment Termination Dates for all employees if possible by her IT team by tomorrow. New COBRA test files need to be provided to Tango if she can extract that information for WT team.
I took a look at the first 10 rows on the report that Scott sent over after the call with the 8/31/2016 termination date. There appears to be issues as some are reporting that were never Benefit eligible as they have a Per Diem Status all of 2016 and others do not have accurate termination dates reporting.

For COBRA Processing:
As per the below comments , its looks like BSG do not have complete data for cobra employee in WORKTERRA. So before processing the COBRA Files we need to discuss and make sure that the data is loaded in WORKTERRA correctly.

Here are updates based on our call today:
· Some benefits eligible but not enrolled are showing correct dates and some are not. This was the adjustment made in the last round to make sure eligible but waived employees were on the file. Some examples…
o A79532 – Karen Hanley – She shows as benefits eligible and opted out from 1/1/2016 through end of year. Kim validated this is correct.
o W408571 – Kayla Arenz – She shows as benefits eligible and opted out from 9/17/2016 through end of year. Kim said she had been eligible and opted out since 1/1/2016.
1/19 notes: Van believes this is a transition issue of the data and the manual addition of the of the opt out date into the system, but it might have only added the later part of the year and missed the earlier part of the year. Kim believe this is someone who was benefits eligible on 1/1 and then moved to a non-benefits eligible position mid-summer, then became eligible again after the move to Workterra. The 9/17 date is still questionable since it should have been on the first of the month.
HSPL: This issue is resolved and these employees are now populate their benefit eligible date as applicable.
· Many employees have a 8/31/2016 benefits termination date, but employment termination date is either missing or it’s 9/9/2016 on the active file. I provided a list of these yesterday to review.
1/19 notes: Still needs to be researched. The development team with Workterra was provided the list of 2016 Employment Termination Dates for terminated Wheaton employees to update their COBRA RET files. The biggest issue is that Workterra does not house the COBRA enrollments in the systems. BSG stores these enrollments in their Travis systems. One option is for BSG to load those COBRA enrollments into Workterra (into terminated medical plans, which is another issue) and try to get those enrollments in the Tango format that’s needed. The other option is for BSG to try to find a way to get their COBRA enrollments into the Tango format that’s needed.
· Missing termination date – Kim mentioned as we reviewed there were some employees missing a termination date. I think this tied to the issue above.
1/19 notes: Still needs to be researched. Workterra did not have the term dates, but the Employment Term dates that Kim provided were given to the development team to see if somehow those can be plugged in.
HSPL: we have loaded the terminated employees in the separate test Wheaton company and we are processing the ACA files form that Test company [Fresh replica of Wheaton].If you want to load these term employees in main Wheaton company Please approach Amit.
· Benefits Eligibility Date was 2 days before hire, but really wasn’t because it was updated. Some examples…
o W709274 – James Murphy – He shows as hired on 9/19/2016 but benefits eligible on 9/17/2016. Later records try to move that to 11/1/2016 (we only move dates if they’re within 30 days of eachother) - Development team please look into this discrepancy and advise where the 9/17/2016 date came from. Please revisit the New Hire effective dates in Workterra to determine the Benefit Eligibility Dates, on the files.
1/19 notes: Need to figure out where the 9/17/2016 came from. Van says maybe it’s the logic in the file and even though it corrected in a later file, unclear why it came through.
HSPL: We have fixed this issue and now all new hires are getting benefit Eligible date as per their new hire/Rehire rule so this employee will get benefit eligibility date as 11/01/2016.
· Per diem exclusions...but that were offered COBRA…need to be communicated. – Per Diem employees should show up on all of the Active DEM files as well as the COBRA RET files since they are all COBRA eligible after their pass their new hire window. However, BSG/Wheaton needs to provide the COBRA data from their Travis systems in order to find out who is Active versus Terminated as well as which dates they were enrolled in COBRA medical coverage.
HSPL : As per the yesterday’s communication we need to exclude these Per Diem Employees from ACA files and today again we have received the requirement that we need to include these employees on ACA files for Active and COBRA. Please help us in stabilizing the requirements well before time.

Kim and our team are also looking into how COBRA eligible employees that are in Active Employment statuses are being reported. We also do not administer COBRA for Wheaton, but are trying to figure out a way to get them reported properly on the Tango files.

[EBS] - The fully insured plans fell off the reports as well

[EBS] - Kim is auditing the files. She is stating that all employees except one employee is listed as full-time. Why are there not more part-time/per diem status
Rebecca Anhalt is listing on the files as full time and she was a part time employee this is in column T (employment status) of the spreadsheet.My guess is that you are pulling this value from the WORKTERRA employee type field. For Wheaton this data is in the UDF “Wheaton Employee Type”
[Harbinger] - Yes, we did the same customization. We will use “Wheaton Employee Type” while exporting files.

All of the plans on the report should be included except the COBRA plans because there are not COBRA enrollments in the system.
Please change the employee type to the Wheaton Employee Type.
Another issue the record count between Aug and September dropped. Aug was 14565 September was 12565. Why was there a drop. We are thinking that the per diem employees are the ones that dropped off the file.
Please advised why the record count dropped. Once that it determined we need to run the files out and compare it to what was sent to Tango and we can start working on the correction files.

We have done analysis of difference is employee count for Aug and Sep month. We have found below major reason for count drop.
1. Almost 14 medical plan has terminated on 08/31/2016 and almost 1000 employees belongs to those plan is not eligible or enrolled in any other plan in Sep month.
2.Another reason is almost 500 employee has terminated in Aug month.
3. Also have big count of employee is not eligible for any medical plan in Sep month.

I need a list of the employees that dropped as of September and the plans they were enrolled in. We need to audit the list of 1000 employees and the 500 employees mentioned above.
Right now the COBRA file is on hold until further notice.
There is total 2288 employee difference in Aug and Sep file. We have done analysis of these dropped employees and sheet uploaded on FTP.

1 "EMP_List_Eligibile_Plans_Terminated_in_Aug" tab contains 1586 employee whose medical eligible plans are terminated on 08/31/2016. Hence these employees dropped in Sep month file.
2. "EMP_List_Not_Eligible_Complete_Sep" tab contains 113 employee those are not eligible for any medical plan for complete Sep month.
Hence these employees is not populated on Sep month file.
3. "Employee_Terminated_Aug2016" tab contains 488 employee terminated in Aug month. Hence these are not populated on Sep month file.
4. Remaining 101 employee is not eligible to any medical plan in complete Sep month.Hence these employee did not populated on file, these will get populated in later months files.
Note : "Plans Terminated in Aug" tab contains list of medical plans terminated on 08/31/2016.

During our call today there were some items discussed that I we needed for Workterra to produce the monthly files for 2017.
Plan IDs for 2017 will change. They will be WHDHP and WPPO. I have attached the 6 Medical Plan names in WT to provide any clarification needed.
There is another area that will need to be programmed differently and possibly discussed regarding Field 31 on the DEM file. In 2017, the Ascension plans moved to a Premium Structure based on Salary bands. 4 salary bands were built for 2017 and associated rates were also built in the WT system to determine the proper premium based on the salary. The Minimum Coverage Amount value will be based on these premiums driven by Salary band, but it will only populate the value for the Single HDHP in that salary band.
For example, someone enrolled in the Family PPO plan for January 2017, earning $34,000 would be in Salary band 1. Their monthly premium in WT would be $335.83, but the amount that would need to report in Field 31 would be 80.17 (single HDHP premium) – HDHP Less than 35006 Sub Rate Name, Employee Pre Tax Cost Associate Only Health Tier.
It gets a bit more complicated when the Ascension Socially Just (Premium Assistance) program comes into plan. This is a program providing a rate reduction (25%, 50%, 75% or 100%) which is captured in a UDF at the Employee Demographic screen (2017 Medical Subsidy), this reduction would then need to be applied to the above amount. There are approximately 375 employees in this Socially Just program.
These changes are very different from the 2016 data, which was a static amount. Can you let me know your thoughts on this and if this would be possible in development? Since we are only providing this report for 4 months, if there is a better way to do this due to development process potentially being extensive, please let me know.
I am still working to finalize all of the rates for those in the Socially Just program. I do have the rates for those based on the Salary Bands

Band 2 35006 to 75006 $112.67
Band 3 75006.01 to 150010 $147.34
Band 4 over 150010.00 $179.84
Please let me know if you would like to discuss this to make sure you have a full understanding of this project for 2017.

1.We have considered few employees whose class 3 is WI for our analysis.
Could you please update the expected EE cost in attach file so that it will give us clear picture .
Let us know, we will need this data for all classes or any specific standard population.
2.Is there any chance that we will need files for all 12 months of 2017 or only for first 4 months of 2017.
3.Let us know, if we have any idea about delivery date of the files per month.
Currently system will display "Minimum Coverage amoun"t on file as mentioned in document.
Once we will get exact expected EE cost for these employees with comments if any, we can give clear thought about any development needs or we can achieve same with customization.

Per Kim at BSG:
I have attached the spreadsheet with requested information along with additional examples for those associates that participate in the Socially Just Program, which is identified by a UDF – 2017 Medical Subsidy on the Demographic Page. The Minimum Cost of Coverage for these associates will be based on an Associate Only PPO rate for Band 1 with the corresponding percentage reduction applied.
Please see the examples provided on the spreadsheet to make sure they make sense to you and the development team.
Also Files for ACA reporting will be needed for January 2017 – April 2017 only. We will need a DEM and DEP file for each month. The files should be provided to Tango, the ACA vendor no later than the 10th day of the month following the end of the month.

February file due March 10th
March file due April 10th
April file due May 10th
We have obviously missed the January due date based on today, Thursday, February 23rd being the date that EBS received the data for the Minimum Cost of Coverage field.
