• rate for above plan - add 2 billable self insured components - assign cos t to them
• map rate and plan
• enroll an employee to this plan eff from 01/01/2016 [date time stamp for this enrollment is sa current date 10th Nov 2016]
• generate an invoice for month of Nov 2016

Invoice should give billing adjustment details from month Jan to Oct 2016 [if addition of those two self insured components was say100 - then cost in the billing adjustment section for each month should be 100]

When same done with billed rate - invoice shown billing adjustment details as per the expectations
