Not working as per the expectations
Follow the mentioned steps
Invoice gives the billing details but no billing adjustments are generated
If you again modify the same rate and mentioned EE ER cost along with the billed rate- report starts giving you the adjustment details too !
Also in the case of multiple Self Insured components mapped to a rate - no adjustments details shown

In case of no EE and ER cost / Billed rate mentioned
Billing and Adjustment (first invoice) are coming out
As per the second comment mentioned above - in case of multiple self insured components : Billing details were shown but not the billing adjustment details.

Have verified mentioned scenarios for Health plans using Billed Rate, Self Insured components and both.
Tested on WT Stage - 26th Dec 2016'
Worked well in the case of no ee and er costs and mentioned billed rates in the mapped rate
System showed the proper costs and adjustments
