SBM - Please look at this tonight.
1. From January 1, 2016 – May 31, 2016, SBM was using the W2 safe harbor. Employee monthly premium contributions were 9.5% of gross hourly pay. Then beginning June 1, 2016 – December 31, 2016, SBM used the rate of pay safe harbor. It appears from my first glance at the revised forms that you show W2 safe harbor for the entire year.
2. Because we used the W2 safe harbor for the first half of last year, employees who earned less than $11.33 per hour, would not have paid the entire $140 employee monthly contribution. (Please see Brude Abathan’s 1095 C Form).
