        Hi Jennifer Leugers

        We have run "2016 1095C for FRMC" report. Please refer latest run for verification.
        We have changed the eligibility rule for FRMC EIN.Please verify in EIN classing Mapping tab under ACA Customization.

        Note: EIN is not mapped for the eligibility rule used in this report.So in 1095C report,Employer name and EIN will display details present in Company Information. Please add EIN's from EIN Classes mapping tab in ACA Customization.Reports need to be re-run after this change.

        Alecto - Fairmont Regional Medical Center (FRMC) has been split from Alecto Healthcare. Please make customization changes on FRMC Company.

        Note: Plans that are not marked as Minimum value will reflect the 1F code on 1095C report.
        Also As per IRS guidelines, Void Box is not to be marked.

        We have used existing ACA Customization for this run.Please verify the same on this company and please make changes on this screen if necessary,so that the report can be re-run with latest customization.
        Also verify the Eligibility rules on 1095C report.
