        County of Lake (COL)

        Because life insurance is included, they have to include the cost of benefits without flex credits.
        Current costs are showing 721.72 for EE’s enrolled in EIA Health Plan.
        699.99 for EE’s enrolled in CalPERS Health Plan

        Because of the bundle, the cost should be 770.20 for EIA Health Plan (ABHP, PPO 80, EPO, PPO Law Enformcement)
        And
        Should be 747.48 for EEs enrolled in CalPERS Health Plan (Pers, Care, Pers Choice, Pers Select, PORAC.

        1. We’ve been told by Alliant’s Compliance team that we need to indicate the cost of our entire insurance bundle, as we bundle life, dental, vision, and health together. Because life insurance is included, we have to indicate the cost of benefits without flex credits. Currently, those costs are showing as 721.72 for employees enrolled in an EIA Health plan, and 699.00 for employees enrolled in a CalPERS health plan. Because of the need to include the bundle, the costs should be 770.20 for employees enrolled in an EIA Health Plan (ABHP, PPO 80, EPO, PPO Law Enforcement), and the cost should be 747.48 for employees enrolled in a CalPERS health plan (Pers Care, Pers Choice, Pers Select, PORAC).

        From what I recall from last year, we completed a spreadsheet and indicated the lowest cost health plan available to our employees. This led me to believe that there is a setting within WorkTERRA that can be changed to reflect those amounts. I was not given a spreadsheet to complete this year.

        Some (but not all) of our terminated employees are showing an increased cost in premium for their month of termination. Benefits terminate at the end of the month in which an employee terminates.

        As an example, if an employee terminates on March 1, their benefits terminate on March 31. Some of these employees include: Scott Abbott, Audrey Knight, Lorna O’Kelley, Lester Hoskins, and Erika Nosera. Some employees that have the correct rate in their month of termination are: H Matt Perry, Gretchen Mills, and Zohreh Lahey.

        EE lowest Cost in ACA Customization can be set against the plan to overwrite the existing EE cost.
        Cost mentioned in above comment need to be set against the plans in ACA customization under EE lowest cost column.
        Path to set EE lowest cost : ACA Dashboard -> ACA Customization -> ACA Customization Tab
        After the costs are set,1095c report need to be re-run to reflect the changes.

        Please refer to the highlighted text in the attached screenshot taken from IRS Site.

        In short it says that,on line 14,an employer offers health coverage for a month only if it offers health coverage that would provide coverage for every day of that calendar month.

        Consider example of employee Scott Abbott, EE loses eligibility/offer of plan "ABHP - Active" which was providing cost $721.72, on the day of termination. As eligibility/offer is lost before month end,EE is not eligible for this plan in the month of termination.

        Next minimum cost affordable plan which is offered for whole month is "PPO 80 Actives" whose EE cost is $785.98

        In case of EE H Matt Perry, EE loses eligibility/offer of plan "ABHP - Active" which was providing cost $721.72, on the day of retirement(04/01/2016). Next minimum cost affordable plan which is offered for whole month is "ABHP - Retiree -65" whose EE cost is $721.72. So there is no difference in the cost in the month of termination.

        Hi all - we have another group that has different lowest cost plans that need to display by class. I know we can do this but I am running out of time to learn it (and you guys are so good at it)

        Those in unit 16 (LCDSA) are the only employees that can enroll in CalPERS health plans. Every other Unit (1, 2, 3, 4, 5, 6, 7, 8, 10, and Unknown) are the ones that can enroll in EIA Health plans.

        Created two rules on below mentioned path to acheive this.

        COL - The pricing is correct for all individuals. However, it does not look like the coding that was corrected before the pricing update has taken.

        Made code corrections 1.23.17 again, please re-run.

        COL forms ran correctly, and 2 corrections will not produce correct 1095Cs.

        1. Jennifer Leugers removed Waive ACA Plan as an eligible plan from the customization screen.
        2. Majority of the forms ran correctly.
        3. There are two employees that need to have codes changed and changing them in WT isn't taking the changes for these two. I need to know if you can change these two 1095C's and then I will run the 1095C report. Attached are the two that need to be corrected.

        As per IRS gov rule - Line 15 will show an amount only if code 1B, 1C, 1D, 1E, 1J, or 1K is entered on line 14.

        As the employee (Christina Orozco) has 1A for line 14 (Nov & Dec), hence the line 15 will be blank for that particular month, even if you do correction of that employee.

        For employee Anarosa Solorio we have made the changes and Re-run the report. Please refer latest run
