1. The forms reflected Davis Moore Automative as there is no report for Davis Moore Real Estate.
Please follow below steps to ensure that their forms reflect Davis Moore Real Estate.
- Please create an eligibility rule for this class.
- Set EIN for this eligibility rule in EIN classes Mapping.
- Create new IRS report with this rule.
- Save and run the report.
2. From IRS Site :
Plan start month: Enter the two-digit number (01 through 12) indicating the calendar month during which the plan year begins of the health plan in which the employee is offered coverage (or would be offered coverage if the employee were eligible to participate in the plan). If more than one plan year could apply (for instance, if the ALE Member changes the plan year during the year), enter the earliest applicable month. If there is no health plan under which coverage is offered to the employee, enter “00.”
Plan A PPO Plan - 04/01/2015
Plan B PPO Plan - 04/01/2015
In this company,medical plans start date is 04/01/2015.These plans continue in 2016 as calendar year plans.
Earliest applicable month for these plans for this year(2016) for an employee is January
For plan start month the date should actually be the plan year start date. This means all Calendar year employers will have ALL 1095s populated with 01 and all fiscal should be whenever their plan year starts.
3. Observations for the common issues found:
When EE are in a limited Non-Assessment or Waiting period, the code in Part II, Line 16 goes from 2D, back to 2A and then 2D again because as per the system data their employment date is set to one date and Effective date is set to some other date.
Ideally,whenever EE comes into the system,his Employment date,Status change date and Effective date should be same.
For eg: EE Kenneth Gebrath
Employment date: 10/10/2016
Effective Date: 11/30/2016
As this Employee came in October,codes for that month is 1H-2D.
But he is not effective until 11/30/2016. So 1H-2A displayed for Nov month.
For EE: Nadar Ali
Employment date: 06/01/2016
New hire waiting period rule is to First of month following or coincident with XX days from Date of Hire.
As this employee was hired on 06/01/2016,he is effective from that day itself as per new hire rule.
As per the data in the system,the codes are correct.
Please correct the data in the system to avoid such issues
OR
1095C form can be corrected from "ACA 1095C Corrections" page.
Path for "ACA 1095C Corrections" is : Log in -> Select Company -> BenAdmin -> ACA -> ACA Customization -> ACA 1095C Corrections
Once you do the corrections for EEs from attached PDF, re-run 1095C reports by selecting flag "Apply User Correction = YES".

4. For EE : *Nedra Tyree *
EE was eligible for Plan A PPO Plan from 04/01/2015-03/31/2016 and Plan B PPO Plan 01/01/2016- 3/11/2016.
Plan A PPO Plan's minimum EE cost is $200 and Plan B PPO Plan minimum EE cost is $104.
Plan B PPO Plan is not eligible for the whole month of March as she was terminated.
So the next eligible for that whole month is Plan A PPO Plan.So the plan cost changed from $104 to $200.
