        We are able to run report "2016 1095C for Matt" by removing "Legal first name" field from order by list.
        Please refer the latest run for reference.

        Development team will look into this problem,
        till then you can run "2016 1095C for Matt" without adding "Legal first name" field in order by list.

        We have re-run "2016 1095C for Matt" adding "Legal first name" field in order by list.
        Please refer latest run for verification

        Also, I have changed the eligibility rule to "All Status Rule" in EIN Classes Mapping to match the eligibility rule set in 1095C report.

        Please research this:
        For the column O (excel file). From what I can see it looks like anyone hired before their plan year start in July the column says January and for anyone hired on or after July 1, that column says July. Examples of those differences are Valerie Yost and Holly Wilde.

        From IRS Site :
        Plan start month: Enter the two-digit number (01 through 12) indicating the calendar month during which the plan year begins of the health plan in which the employee is offered coverage (or would be offered coverage if the employee were eligible to participate in the plan). If more than one plan year could apply (for instance, if the ALE Member changes the plan year during the year), enter the earliest applicable month. If there is no health plan under which coverage is offered to the employee, enter “00.”

        In this company,medical plan start from 07/01.

        These plans start from July.So earliest applicable month for these plans for this year(2016) for an employee hired after July is July.

        For plan start month the date should actually be the plan year start date. This means all Calendar year employers will have ALL 1095s populated with 01 and all fiscal should be whenever their plan year starts.
