Upload Document functionality is not working correctly.

      Description

      In Stage Branch, during employee pre-board tour, Upload Document functionality is not working as expected. Below issues observed:
      1. After selecting file of allowed format through browse, file name is not displayed in input box.
      2. No message displayed on Upload/Save button.
      3. Uploaded and saved document is not displayed on 'Supervisor Approval' screen of level one admin.
