          Good Morning Michelle and Debbie,

          We are seeing a problem with changes to spouse voluntary life and AD&D coverage for Dana Neiswander (Spouse Caleb Brown) that seems to have been triggered by an employee data change.

          The email string that I am forwarding has all of the history.

          I am auditing the voluntary life elections. I will let you know if I find any others.

          Please take a look at this issue as soon as you can.

          It seemed odd that her husband’s life amount matches her Basic Life at $87,000. There’s an issue with reinstating his coverage. Dana carries $500,000 of Voluntary Life, but I only see options for him up to $87,000. This will need to be corrected.

          The way the plan was built, the spouse election is 100% contingent on the Basic Life and AD&D Benefit Group E. Where it should be 100% contingent on the Employee Optional Life Group E. Can you please run an enrollment report to see who is enrolled in the EE Optional Life Group E AND the Spouse Optional Life? Once you have this information let me know.

          Client Michael Best Benefits has a plan Spouse Domestic Partner Optional Life- Group E the plan eligibility is contingent on the EE Optional Life- Group E plan. However, under the Cost and Coverage tab, the Determine Dependent Max Coverage Amount By Employee Election is set to 100% and Plan Design for Dependent Contingency is set to "Basis Life and AD&D Benefit Group E" the correct plan should be the "Employee Optional Life Group E".

          Can you please update this? In additional can you please verify how many EE's this may be affecting.
