          We have verified medical deduction for Jowayne Jackson for FGO HDHP Plan with tier Employee + Family:-

          As per current customization on rate (2017 HDHP), Employee + Family tier has cost 1438.09.

          Please refer below Cost Calculation for FGO HDHP Plan :-
          Employee Monthly Cost is 1438.09
          Payroll Schedule is Bi-Weekly(26)

          Employee Annual Cost =1438.09 * 12 =$17257.08
          Employee Payroll Schedule cost= 17257.08 / 26= $663.73

          Cost on Export file =$663.73

          Regarding Employer Define contribution employee enroll in FGO HDHP Plan with tier Employee + Family and this tier covered EE + SP + 2CH and we found that tier has no cost on Employer Define contribution configured.

          In Employer Define contribution cost is configured for only for EE + SP + CH (Employee + Family).

          Please let us know in case of any issue/concern.

          ALL - I created the Defined Contribution for every tier that was in the system that is used on the rates. How could we not have that tier if there is a rate? This isn't making any sense.

          Really need someone to look into this ASAP.

          Above suggested tier was not present earlier. Tier added on Defined Contribution by 'Jennifer Fernandes' partner on 1/20/2017 and also recalculated for 'Jowayne Jackson'. PFA screen shot.

          Please let us know further action item on this.
