ACA - JAFRA - ACA Setup details

Request you to provide below details in attached JAFRA ACATool Configuration.xls format

1] EIN
2] Contact details for each EIN
3] Total 10 Medical plans which needs to be marked as Minimum Essential coverage, Self Insured, ACA Plan, Is Plan Minimum Value, EE Lowest Cost, Plan Enrollment Start Dat [Only for non calendar yer plans, started in 2015].

NOTE 1] below Two medical plans provided in excel JAFRA WORKTERRA ACA Tool Configuration v2016-02-03 LK.xlsx are not available in JAFRA Production.
1.1] Anthem Classic PPO 250/20/20
1.2] Anthem Value Hmo 20 40 250 3 day

Attached excel JAFRA Employee Census.xls has all employee from JAFRA production with employee type as full time.
Let us know if any employee type is not correct in system so that we will correct it.
