I had a call with BCC today and the example above is correct. It is ok that there may be many rows on the summary due to adjustments. The summary should have a total for each benefit type.
It is ok that the volume would be blank for non volume based benefits. But please remember the volume need to represent the amount that the rate is based on. Either coverage or covered payroll
