Michael Nicholls: Hired 12/8/2015 Jan and Feb should be 2D
Waiting Period is First of Month Following ( or coincident with) 60 Days from Date of Hire – Would 2D be the correct code?
Sean Vander Zanden: Hired July 6, 2016. July, August and September should be 1H/2D.
I looked at the record and I see the EE waived medical for 2016. Based on Waiting Period, would be 2D for August and September.
Sarah Mendoza: January through March should be 1H/2D. She was hired in January and not eligible for our plan until April.
DOE January 4, 2016; Benefit Eligible 4/1; Why wouldn’t she be 1H/2D for those months.
Jesse Larsen: Never actually became an employee. Should not have a 1095 at all.
DOE 5/31/2016, EE terminated eff 5/16 on 5/23 by the Admin; system is picking up one month of limited assessment due to the May hired month. What should the system doe when someone is hired as never effective? How should this term record have been processed?
Andrew Vannieuwenhoven: Never became an employee either.
DOE 9/20/2016; EE Terminated Eff 9/15 on 9/20 by the Admin; system is picking up one month of limited assessment due to the September hired month. How should this term record have been processed?
Eric Saft: Hired but never showed up.
DOE 01/18/2016; EE Terminated Eff 1/19/2016 on 1/22 by the Admin; system is picking up one month of limited assessment due to the January hired month.
Birket Sullivan: Hired 01/01/2016 Feb and March 1A/2H should be 1H/2D
DOE 1/11/2016; Benefit Eff 4/1, so Feb and Mar should be 1H/2D; Why did the system not pick up on that? Is there something we are setting up right?
Matthew Tremble: November discrepancy. Initially hired as PT. Transferred to full time as of November 1st. 1H 2A incorrect
DOE 11/7/16; Benefit Eff 2/1; EE was employed as PT employee but PT are not tracked in the system. Code 2A says EE was not employed but they were just not in our site. What do you advise clients in these scenarios? Edit the form to reflect the correct code? I don’t see how the system is going to reflect the correct code for this type of scenario.
Owen Aerts: Hired in Feb but was not full time until June. Feb, March, April and May should be 2B.
DOE 6/13/16; Benefit Eff 9/1; in the system we do not track PT, so the system is going to base the ACA code based on when the employee was added in the site and is assumed to be a FT EE. Would you have her update the form manually?
Aaron Mueller: Term 10/28/16 October 1H 2D should be 1H 2B
DOE 3/27/1995; Term date was first processed on 11/4 for 10/21/16, then the Admin came back on 11/21 rehired the employee with a rehire date was not set, but eff date was changed to 10/28/16. He was not an EE for the full month of October so wouldn’t line 16 be 2B
Travis Reiland: FT to PT 1/4/16 January 2016. 1H 2A for Feb and March not correct
Need guidance on how to handle these records where we do not track PT status
Ryan Eiting: Hired in May 2016, terminated in July 2016. May and June should both be 1H/2D.
DOE 5/2/2016; Benefit Eff 8/1; Limited period May and June would be 1H/2D. May does show the 1H/2D code but June does not.
Amy Kue: Hired on 2/1/16 April is 1A 2H should be 1H 2D
DOE 2/1/2016; Benefit Eff 4/1, Is the system showing the 1A/2H code because for the waiting period rule being Waiting Period is First of Month Following ( or coincident with) 60 Days from Date of Hire?
Erik Fanning: Hired in April, termed in August. April, May and June should be 1H/2D.
DOE 4/11/2016; Bene Eff July1; so April, May and June would be the limited assessment period code 2D.
