Can we add 3 columns (Title on this page, Action required attachment, Reference attachment) in Document or should we create different table using documentid as foreign key?

You can use anything. We can put text and if anybody click on that all the forms get selected

Finalized the Send document DB structure and Rushikesh created these tables.
This contains attachment data structure too.

Done with this page today with backend jquery logic. some checkbox part causing an issue with labels is left we will look into it tomorrow. moving with backend API for this page.
