            Below new fields are added in this customization-
            -Consider Taxable.
            -Maximum Taxable Amount.
            -Consider Coverage End Date having 1 dropdown for its rule and textbox for number of months.
            -Maximum Employer paid coverage.

            Corresponding business logic jira is NF-199
