I have checked the template, it has worng date mapped please refer attached Effective date should be mapped to employee formdata effective date.

There were duplicate Additional form entries in system. This was causing the failure of Import. We have corrected the data and Import will be successful now.
We are looking into why those duplicate entries were inserted and will update you accordingly.

Hi Swapnil Pandhare,

There were duplicate entries in Employee_Formdata for 4 employees. When checked, we found those were inserted by employee login. We have corrected the data. The duplicate entries are present on HSPL copy or we have also taken a backup before correction.

Can you please take this ahead.

Adding comments for clarification from Satya - a dev fix is still needed to prevent duplicate entries from entering the system.

Adding 3c' for import issue

Concern: Import is getting failed to execute.

Cause: There were duplicate entries against employee form. Causing exception.

Correction: We have removed the duplicate entries. Now import is working fine.

PS: Duplicate entries found to be added by UI. EDS team is working on that.

We have tried to reproduce the issue.But not able to reproduce the issue.
Please refer below scenarios that We have tried,

1)Tried with Both OE and New hire mode.Added new additional form.Verified through employee login.
Data saved properly.No multiple entries found in history table against newly added forms.
2)Verified with requires verification set on modes.
3)Tried to click on multiple times on Save and Continue button but on first click only,button gets disabled.
4)Tried to skip the additional form

Please check this issue at your end.

Verified below scenarios on Clarke for hspl production but its not reproduced at our end through import.

1) Imported additional form data through import. - import done successfully
2) Updated additional form data through import- import done successfully
3) Traverse the OE and new-hire mode
4) Tried to save and continue additional form multiple time, no duplicate entries found on UI.
5) Added entries of Health Care for two Married Employees, Sick Leave Balance, State Health Website, Tobacco Usage through import at same time and updated.
6) In workflow saved all the forms by updating through UI, no duplicate entries found.
7) For adding additional data and updating through import, import done successfully.
8) Verified for single and multiple employees.

Verified to reproduce the duplicate entries issue on Production on Hspl copy but could not do so. I tried this through the New Hire Mode,OE Mode and also through QE mode. Also tried it through approval but could not reproduce it.

Issue of Import failed was due to duplicate entris in Employee Form.
It was corrected and import worked fine but this issue not getting reproduced at our end.

We will keep track of such issues in case it occurs again.
