          Hi Rashmita Dudhe,

          As per discussion with Swapnil, for additional forms we are not adding an default constraint,
          and this is existing system behavior.

          We did the analysis and found that this is implemented for trac (#11403 : The Wellness flag on the
          demographic page for Austin employees defaulting is not working) and this is deployed on production on 25th March 2015.
