#1 ) Concern : The Vacation Hours Available for Sale form shouldn't be in the employee flow. it doesn't seem to be working.
Cause : For this form, EmployeedatascreenModuleMapping entry was missing due to server error on add form. We will update you on root cause why this table's identity was reset.
Correction : We have reseeded the identity for above table and added missing row for this form. Now this form is visible in Employee data screen and can be added in workflow.

#2) Concern : The Tobacco Usage - can't add the form into the Employee flow in Employee Data Screens as it isn't showing to allow selection.
Cause : For this form, EmployeedatascreenModuleMapping entry was missing due to server error on add form. We will update you on root cause why this table's identity was reset.
Correction : We have reseeded the identity for above table and added missing row for this form.

Now this form is visible in Employee data screen and you can add in workflow.

The Identity reset on tables was done during DATA load on ATC company on production.
The reason for identity reset is updated by Jaideep Vinchurkar in Jira Ticket https://workterra.atlassian.net/browse/WT-12021 .
