Hi,
This client is currently in OE. We are seeing that there are a couple of people who are showing dual coverage on their confirmation statement and we are not able to see what is causing this or how they were able to get this coverage.
It looks like the employee initially chose the coverage, but then there is another event that happens on 10/20 that is enrolling the employee into that second coverage.
For instance, Andrew Surita - 2017 Dependent Care FSA - he is showing that he had elected $100 on 10/19, however, on 10/20, it is showing that Marianne did a data change (which we are not able to see what triggered this data change) which is now causing the member to show 2 enrollments for 11/1/17 - 12/31/17. We believe that he should only have the $100 coverage.
The other member that we saw this on was: Jody lahr - who is showing two vision plans, one of them is dropping one of her children, Grace. Her child Grace should not have been dropped from the coverage.
Chun Lee also has 3 FSA lines of coverage.
Could you please review Rogers and help us with the following?
- Is there is anybody else impacted?
- Please correct these two people's records.
- Please let us know what caused this
- Did this impact any other clients?
