Intrepid USA Basic Life and Basic AD&D Effective Date Issue

Employees have an incorrect effective date for Basic Life and Basic AD&D (linked issues)
I am reviewing the 1st 2 records. It seems like on 10/1/17 eligibility rules ran. I am going to review with Dev on what caused the change but here are some conflicting items I have seen so far:
Example 1 - Christy Aubrey
The employee had 2 class changes (One on 5/11/17 and 8/24/17) I notice that there are 2 event builders that would have triggered the same QE. I have not seen this cause an issue but wanted to point this out.
Also - on 10/1 (effective date of the new benefit), the plan was termed back and re-enrolled based on the effective date of change.

When reviewing the plan and other rules, I noticed the following:
1. Plan - Term rule is "Date of plan termination"
2. Plan - Newly eligible rule is "1st of the month following 1 month from DOC" ----UPDATED on 9/13/17
3. Company - Company rule is End of month Term / Effective Date of change
4. QE Rule - 1st of month following 1 month from event date
I have a feeling that the rules are conflicting with themselves. I will check with Dev tomorrow to confirm
