Disys - Servor error when viewing employee view history & HSA responses

Hi,
We are getting server errors when trying to view the history of the employees record for the Optum HSA Affidavit custom form.
Could you please help to resolve the error?

Also, it looks like the following people used to have a response on this form, however, the responses disappeared. Would you be able to let us know what their response originally was on this form and place it back? Also, please let us know why the responses were deleted from the site.
