University Health - Tobacco form not saving in employee workflow

We are having an issue with the Tobacco form not saving when employees go through the workflow. Employees are answering the question and hitting save and continue but they are not getting the tobacco credit if they select "no"

The admins/partner users are having to go in can click save again on the form to get the credit to show.

Two employees that they had to fix were: Jessica Bennett and Christy Lummus
