what is the correct date to change the class? You have mentioned only one date 11/30 in this ticket.

The date should be 11/30/2017

Employee class change done with wrong effective date. We have have to change the effective date from 11/12/2016 to 11/30/2017. We have to revert the enrollment changes too

For "Flavia Gagin", her "Class 2" was changed to "55 Ineligible" on "2017-11-21 08:54:45.843", hence all enrollments were termed in 2016 only. There are no active enrollments in 2017. Please find below details.

Please confirm if you want to change termination date of all above enrollments to 11/30/2017.

Please note : If you want to change term date for above enrollments to "11/30/2017", then as per current customization on "Group Long Term Disability Class 2" plan, its term date should be "11/12/2017". Please advice.

When the class was change back to 2016 her 2017 benefit were termed as never effective. Is there any way to look in history and see those terminated benefits?

FLAVIA GAGIN had following 4 elections in 2017 which are terminated as never effective at the same time when her class 2 was changed to "55 Ineligible"

Do you want to reinstate above enrollments with termination date 11/30/2017?

If Yes then do we need to change termination date of earlier given 12 enrollments to 12/31/2016 so that 2016 and 2017 enrollments will continue. Please advice.

You reported 4 enrollments that with effective date of 1/1/2017 which were termed as never effective when class change made the EE ineligible as of 11/12/2016.
You have stated that these could be reinstated with term date of 11/30/2017 as correction. This is fine.

Question: Only 4 enrollments are found to be effective 1/1/2017, so isn't it that only 4 of corresponding earlier enrollments should be terminated as of 12/31/2016 and rest 8 enrollment be terminated on 11/30/2017?
