The above EPT employee elected coverage in 2017; however, his payroll deduction information was apparently not sent over to the City so the City never took deductions from his paycheck. Can you check to see if his elections were in fact sent on the City’s payroll deduction report, or if there was a problem with the information.

We scanned such employees and we found only one employee that did not sent over the files.
Employee : MELISSA FOX. Enrollments added for plans Kaiser, Guardian Incentive PPO, VSP Vision Base Plan on 12/14/2016 with effective date 01/01/2017, But did not export on next file. At that time file criteria was Current Year Election Yes.
Payroll File No is 9161
