Please delete the following benefit types and plans: DELETE Long Term Disability and DELETE Voluntary Short Term Disability

Please remove the following User Defined fields: DO NOT USE Spouse Eligibility Status and DO NOT USE Retiree Eligibility Status. I’ve attached a screen print of the Add Employee screen for your reference. Please note these incorrect user defined fields are in the Form Builders: Add Employee and Demographic Information.
