Hello Development Team,

Carrier is requesting us to remove the coveragelevel EE+SPOUSE for only the Superior Voluntary Vision Plan. There was a miscommunication between carrier and wageworks. They have manually removed this coveragelevel from today's QB file and reloaded it. We won't need to resend them a new file, but they are expecting future files to not include this coveragelevel for the vision plan. Below is their original message for reference:

The QB Import however had incurred one error which upon further investigation, was due to DBS not communicating the proper instructions to Wageworks. The error was connected to the Superior Vision Plan, our rates for this plan do not include a separate rate for QB + Spouse however on the import instructions we had communicated that this was true. The previous instructions to include a separate rate for the employee + spouse (QB +Spouse) was incorrect and we apologize for this. The correct rate bands for the Superior Voluntary Vision are as follows:

QB Only: 11.68

QB + Child: $23.36

QB + Family: $30.99

Spouse Only: $11.68

Please remove the QB+ Spouse for the Superior Voluntary Vision Plan from today’s and future reports for Suburban Electrical Employees.
