As per Satya @ This is currently available with Confirmation statement report, however as discussed with Debbie today, we got to know that for employee level confirmation statement print as well, we need Envelop format. We will schedule this task in Sprint 2 & JIRA ticket updated.

We have a query related to this requirement , to display Address in Envelope Format in Demographics section on Confirmation Statement.

In the Format Confirmation Statement tab, we can select the fields (for e.g Street Address 1,City etc) which get populated in Demographics section in Confirmation Statement.Only the selected fields populate on Confirmation statement.

1)If the Partner/Admin does not select any of the address related field (for e.g Street Address 1,City etc) in Format Confirmation Statement what should we populate in the Address envelop format?

2)If the Partner/Admin selects only one of the address related field (for e.g Street Address 1) in Format Confirmation Statement what should we populate in the Address envelop format?

This enhancement is done. I have made changes in _EnrollmentSummaryEmpInfoPV.cshtml .

Assumption :
SSN,Date of birth and gender should not be disabled from format configuration changes.
If they are disabled then you can see textbox empty for above fields.

I am attaching both the screenshots of above 2 scenarios.

Verified on LB using company DC. Below issues observed :

1. In Print output, 'Social Security No.' is displayed as '---' though data is present and it is selected in demographic customization too. Secondly, SSN/SIN and Social Security No. both are present at a time. PFA screenshot 1.png
2. As per customization on 'Format Confirmation Statement' screen, in print output, Demographic section is placed between pages. Due this, continuity of content is lost and alignment is disturbed. 2.png

UI issues :
3. In 'Demographic' popup, top checkbox is misplaced. PFA screenshot 3.png

Suggestion:
4. On 'Format Confirmation Statement' screen, if check box 'Pages' is selected, popup 'Demographic' opens. Actually, it should open on Demographics checkbox only. Provide seperate button to open 'Demographic' popup. PFA screenshot 4.png

Verified on LB using company DC. Observed issue with customization of combo 'Display SSN or Employee ID' . Though value 'Employee ID' is selected, 'Social Security Number' section is visible. Please display SSN or Employee ID as per selection. PFA screen shot. QA-1.png & QA-2.png
