1. As per customization on 'Format Confirmation Statement' screen, in print output, Demographic section is placed between pages. Due this, continuity of content is lost and alignment is disturbed. PFA screenshot 1.png
2. On 'Format Confirmation Statement' screen, if check box 'Pages' is selected, popup 'Demographic' opens. Actually, it should open on Demographics checkbox only. You may Provide separate button to open 'Demographic' popup. PFA screenshot 2.png

Hello Rajendra Joshi,
#2 On 'Format Confirmation Statement' screen, if check box 'Pages' is selected, popup 'Demographic' opens. Actually, it should open on Demographics checkbox only. You may Provide separate button to open 'Demographic' popup
It is system behavior. If check box 'Pages' is checked, it checks(tickmarks) all the checkboxes and only demographic has popup so it shows that popup.
It is working correctly.

Please keep this in development backlog. We will target it after completing Must have bugs.
