Prime sent the following message today with attachment (password will be sent separately):
I have attached a report of employees that are showing as a compliance risk through our third party ACA vendor Equifax. There are 309 employees that are active in the Workterra system but are not coming over on the IRS Employee file. What this means is that Prime will be at risk for penalties and fines so we need to understand why these employees are not being reported by Workterra?
Please advise at your earliest convenience.

I spoke with Prime today and they have added more information to the message I sent yesterday. The attached report I sent yesterday is a list of 309 employees that are active in Workterra but are not coming over on the IRS Employee file and not showing on the 1095-C forms for 2016.
Please review for 2016 and advise a.s.a.p. why these employees are not being reported by Workterra?

The list shows employees that are missing from 2016. Prime can see them in WorkTerra, but the employees never came over to Prime in an IRS file. Please review and update immediately

Prime reviewed report titled "309 Employees Analysis Feedback." It was discovered that many employees highlighted in orange listed as not in WorkTerra are in WorkTerra. I went through every employee highlighted in orange and listed the location where they work or worked in 2016. Many are with Landmark, which was a Prime_Healthcare facility until 5/1/2016. Others are Prime breakout employees and I have listed them on the last column of the attached report.
Please find attached "309 Employees Analysis Feedback_Updated" attached. Password will be emailed to Mahendra Mungase. These need to be updated immediately so that I can advise Prime that they are fixed as of tomorrow, 2/9/2017.

See attached spread sheet to send over to Development I’m just curious the understanding I have is that the facility notifies you that we will no longer be utilizing your services? As of January the benefits coordinators no longer have access to Workterra so apparently someone took away their rights so why do we still continue to get information on out reporting side?
We have alerted Development and EDI of the facilities that went to Lawson effective 1/1/2017 so that any reporting is stopped.
The count for Centinela Hospital looks really low 848 confirmation.
We have considered Centinela Hospital Facility only & we confirmed the 848 number again. If we need to consider any other standard population please let us know.
There is a discrepancy of 462 Employees between December 2016 and January 2017 data please analysis and advise

The client advised that they are still reviewing the January and February 2017 reports and will have updates later this afternoon. However, they did point out that Southern Regional is still coming across the January and February files. Southern Regional is no longer with WorkTerra as of 12/31/2016. The other Prime_Healthcare facilities that left WorkTerra as well are:
- Garden Grove (breakout as GGH in WorkTerra)
- Dallas Medical Center (breakout as DMC in WorkTerra)

Prime (the client) has provided some feedback on the reporting provided on 10/Mar/17 5:52 AM. Please see comments below:
Prime is looking at the January file for Sherman Oaks and below are the following concerns:
1. There are employees that are on the Workterra system but are not being pulled when you send the file.
- Example Carlos Canas - He is set up on Workterra as an active employee but he is not on the January file as an Opt out. Screenshot below shows that employee is enrolled in Waive Medical Opt Out Plan effective 1/1/2017:
This enrollment missing will cause an error Prime has him as an employee client is not offering him medical benefits.
Client states that they don't believe the files are right as they have over 80 discrepancies on the Sherman Oaks facility alone. Client will try to pull examples in the other Prime facilities for Development to research.
Client believes that they are still not seeing the opt out credits on the January and February files. Please research and add 2017 Waive Medical Opt Out plans to the files.

Client did have an item that they would like Dev to research:
Saint Marys Reno Anesthesia for 2017 now shows none of the employees on WT's reporting exist in Prime's employee files. Prime is asking why they don't have any records of the actual employees that work under the FEIN 300852838.
For example Shane Kruitbosch EEID198069 is active on WT under Saint Mary’s Medical Group but Prime hase him working at Saint Marys Reno Anesthesia.
Karen Willemsen EEID1089 is under Saint Marys Reno Anesthesia FEIN on your report but Prime does not show this employee working for Prime at all. Note: This employee is listed under Summit Surgery Center at Saint Marys Galena, which is no longer with WorkTerra as of 9/30/2016.
Can you please look at these examples and advise the client what is going on with this FEIN? Prime advises that the 2016 files were fine for this FEIN, so can a comparison be run between 2016 and 2017 to see what changed?

[Harbinger] As we process the files who has offer but 'Carlos Canas' employee doesn't have any aca plans offer i.e. Line 14 Code 1H & 'Waive Medical Opt Out ' plan is not marked as ACA Plan.So that the mentioned employee is not expected to be on the file. We verified that the employees who has aca plan offers but enrolled in medical opt out plans are already pulled out on the file. and whoever enrolled in 'Waive medical opt out plans' that employees 'Plan Code Accepted' has been set as blank as given instruction by you in the jira(https://workterra.atlassian.net/browse/WT-663?focusedCommentId=40270&page=com.atlassian.jira.plugin.system.issuetabpanels:comment-tabpanel#comment-40270). [Harbinger] 1.Shane Kruitbosch EEID198069-As per current customization the employee is found on file under the ein-455557052 i.e. Saint Marys Medical Group. These are two classes with different EIN .And Saint Marys Reno Anesthesia class (Reno Tahoe Anesthesiology) has no employee on Prime space healthcare. If you need this employee required under this Saint Marys Reno Anesthesia. We need to change the EIN in backend. These employees are under contract and have a TPA which processes their 1095 C's, so these employees do not need to be included in any ACA reporting for 2017.

Attached, please find a recent request from Prime_Healthcare. Attachment is titled JIRA - WT-8076 with password. The password will be emailed to you separately.
Screenshot #1: The employees listed should be moved over to St. Mary’s Reno Anesthesia. Prime has listed the employees below, but WT needs to create a platform for FEIN 300852838 as it currently resides in Workterra as Summit but has the Reno Anesthesia FEIN# and Summit Employees.

I have audited all employees on the form and have created a new spreadsheet showing only the items that we need Dev to research. This report is titled Dev_UPDATED Report from Prime 6.30. Each of the above records showed that EEs had a QE between January-May. For example EE originally enrolled in 1/1/2017, QE on 2/1/2017, report may be pulling the earliest effective date instead of the true effective date of the original enrollment. So the EE shows 2/1 and gets pulled onto 2/1 and not on the 1/1 report. Can you please see if perhaps this is the reason that so many employees are missing from the January 2017 reporting? Thank you.

I. To populate LOA employee on file we have only below 2 options.
- We will have to select LOA status on all the ACA plans for all prime facilities. In that case all the employee those are LOA will also be populate on file.
- Any employee who is on LOA and again renerolled in the system, he has to select his benefits from 1st of the month.
If you are not agree with the above 2 options then will have to make the changes in code to handle above situations and for that we need Dave approval.
II We have done analysis of file Dev_UPDATED Report from Prime 6.30. except employee of Providence Medical Center employees. Please find the attached Analysis_Dev_UPDATED_Prime 6.30.zip file contains detail analysis of all the employees.
- Analysis of CHMC Employees sheet contains detail analysis of CHMC 52 employees why these were not populated on Jan-2017 file.
Out of these 52, 14 employees were terminated before 01/01/2017 and 18 employees are in assessment period so we are not providing benefit to those 18 employee and detail analysis of remaining 20 employees. - Analysis of HMC Employees sheet contain analysis 7 employees analysis why they are populated on Jan month file.
- There are basically 2 reasons why employees from Coshocton were not populated in Jan-2017 file.
1. ACA plans in company is not marked as minimum essential coverage and self insured in the ACA customization screen. I have marked all medical plans as minimum essential and self insured plans in the system. Please see the attached screenshot.
Please confirm the same.
2.Other reason, There are almost 12 Class1 in Coshocton company,out of these only one Class1 Coshocton is mapped with genericeligibilityrule (i.e. EIN). From the list of employee you provided, almost 61 employees is belongs to other class1 (not with Coshocton).
Coshocton Employee_Class1_Info sheet of attached file contains list of employee whose classes are other than Coshocton, so they are not populating on jan-2017 file. You will have to mapped other Class1 with genericeligibilityrule to populate these missing employees as well.
We are doing analysis of Providence Medical Center employees. Password sent in mail.
Note : It will be more helpful for us if you provide employee SSN while sending for employee for detail analysis.

II. I am checking with Prime Healthcare regarding Coshocton employees. There is only one class with several different facilities listed. I have asked Prime if there is an EIN attached to each facility. I will report back as soon as I have more information.

I. To handle the above condition of LOA to re-enrolled. We have only one way to do it. Though the employee is enrolled or eligible for single day of the month such employee will get populate on file irrespective of its status.
- e.g. Employee XYZ hired on 06/01/2016 and enrollment is terminated on 01/18/2017. This employee will get populated on Jan-2017 month file by considering full month coverage irrespective of any status.
- e.g. Employee PQR is hired on 10/22/2016 and its enrollment effective from 01/22/2017 and enrollment terminated on 04/30/2017. Still this employee will get populated on Jan-2017 file. We will consider eligible or enrolled in ACA medical plan for complete Jan month.
Please confirm the above two examples.

I. We can not handle this only for employee who was LOA and re-enrolled again. Any employee employee who is getting benefits for single day of month will get populate on respective month file irrespective of its any status.
Below are some of the examples of employee which will get populate on file.
- Suppose an employee PQR was terminated and again active in the system with enrollment effective date 01/15/2017. This employee will get populated on Jan month file.
- An employee LMN was on LOA and becomes effective for benefits and enrolls 1/22/2017 then this employee will come on file.
II. I have mapped all the facilities that fall under Coshocton with same EIN 320506896.

- Employee Carl Ling Jr. is enrolled in waived medical plan from 01/01/2017 to 03/02/2017. Carl Ling Jr. terminated on 03/03/2017 in the system. He has an offer of minimum cost plan EPO1 Plan from 01/01/2017 to 03/02/2017. So in Jan-2017 and Feb-2017 EPO1 Plan is populated as minimum cost plan on ACA file. Now if the employee is eligible for single day of the month still we are going to show employee on the respective month file. As he is terminated on 03/03/2017 then what is the expected cost should we populate on March month file. (Should we show cost of minimum eligible plan EPO1 Plan on file?). Please let us know your inputs for the same.

He has offer of Saint Marys Regional Medical Center Employee PPO Medical Plan as minimum cost plan, so files from Jan-2017 to May- 2017 Saint Marys Regional Medical Center Employee PPO Medical Plan is populating as minimum cost plan on ACA file. Now if the employee has coverage of single day of the month we are going to populate it on file. As this employee is terminated on 06/05/2017 then which plan should we populate as minimum cost plan on the June-2017 file either enrolled plan (Saint Marys Regional Medical Center Employee Medical Plan) or minimum cost plan (Saint Marys Regional Medical Center Employee PPO Medical Plan) on June-2017 file? Please let us know your thoughts on same.

While doing analysis to populate employee who is getting benefit for single day of the encounter with one more query in below scenario.
- Employee JENNIFER VAZQUEZ is enrolled in Saint Marys Regional Medical Center Employee Medical Plan from 01/01/2015. JENNIFER VAZQUEZ is terminated on 05/22/2017. He has an offer of Saint Marys Regional Medical Center Employee PPO Medical Plan as minimum cost plan, so files from Jan-2017 to April- 2017 Saint Marys Regional Medical Center Employee PPO Medical Plan is populating as minimum cost plan on ACA file. Now if the employee has coverage of single day of month we are going to populate it on respective month file. As this employee is terminated on 05/22/2017 then its minimum plan offer(Saint Marys Regional Medical Center Employee PPO Medical Plan) is terminated on 05/22/2017 and its enrollment (Saint Marys Regional Medical Center Employee Medical Plan) is terminated on 05/31/2017 as we are terminating benefits on end of the month.
Which plan should we populate as minimum cost plan on file of May-2017?
We are working on the above provided file and let you know once we done with the analysis.

We have had some of the Prime_Healthcare facilities notify us that some of their Active employees are listed under the facility called "Delete ACA." I ran the attached report for Prime_Healthcare and see that there is a substantial amount of Active employees that are listed under this facility. Password will be sent separately.
Can you please provide an explanation? We are not sure why these employees are showing under Delete ACA.

We had shared list of duplicate employee across all the facilities of Prime in WT-6186. List contains employees SSN with their company names. Jennifer Johnson had provided inputs to delete duplicate employee from one of the facilities for all the employee. As we discussed, Instead of deleting these duplicate employees we had created new class 'DeleteACA' and moved duplicate employee in that class. Also while delivering ACA files we are not considering these employees.

We are not ready to move forward with the January 2017 ACA files. The client has not completed their review of the recent HMC issue.
Additionally, it was stated that there are several employees enrolled in a medical plan AND the Waive Opt Out/Spouse Opt Out Plans for St. Marys Health Network and St. Marys Medical Group. Please advise if you are able to run this reporting for both facilities and advise which employees are enrolled in both medical and opt out plans.

I have analysed attached 70 employees, all these employees are enrolled in 'Saint Marys Regional Medical Center Employee Medical Plan' as well as 'Spouse Opt out' plan. While exporting ACA files considering only medical benefits plans. The benefit Type of Spouse Opt Out plan is Spousal Opt Out Credit, We have not considered enrollment in this plan while exporting ACA files.
Also system is not support to delete/remove opt out coverage automatically, so we can not remove its coverage from opt out plan, need to remove it manually.
Please remove the Opt Out Plan manually at your end for all these employee.

2. If employee is eligible for single day of the month, Should this employee populate on file?
[EBS] : We will have to confirm this with client in call.
3. Provides list of employees neither waive and nor enrolled for all the facilities of Prime.
[Harbinger] : Harbinger team will provide list of neither enrolled nor waived employee for all the facilities of Prime.
To get list of neither enrolled nor waive employee for one company need complete one day including verification. In Prime we have almost 15 facilities so we need 15 days to complete this task. Everyday we will try to complete 1 or 2 companies and share report with Amit and onshore team.
4. List of waive or enrolled employee but not eligible for any ACA medical plan for all Prime facilities.
[Harbinger] : Yogesh will provide employee those are enrolled or waived in medical but not eligible for any ACA medical plan for all the facilities of Prime.
We need 3 days to complete this task for all the Prime facilities.

Please find the attached EmployeeList_Demographic_Classwise_EIN_Different.zip file contains list of employees whose EIN on demograpic is different than class-wise EIN. We have found almost 322 such employees across all the Prime facilities processing for ACA.
Attached file contains 2 columns EIN on Demographic (Employee EIN on his demograpic page ) and Class-wise EIN (EIN as per employee class1).
As per our current logic we are giving priority to demographic EIN. On ACA file Demographic EIN is populating for all these employee.
Can you please confirm with client about which EIN are valid/considered for all these employee?

Mr. Rhodes indicated that he would reach out to DEV directly; however, I did want to list the client note below from a 1/31/2017 email for documentation purposes:
The IRS benefit plan should be sent to us with the new 2017 plans prior to the first file being sent as of today we have not received and last year we never received from you at all?
Also we are experiencing many issues in ACA compliance with the reporting that is sent over from Workterra. We need one change to happen on the January 2017 file
that is due on February 10th,2017 for both the Employee and the Dependent file.
On the Employee file Column I “ Benefits Effective date” should be all employees for the month of January 2017. On all other months we should see only the changes made with that month
So on March 10th, 2017 when you send us the February 2017 file the only thing that should be on that file are any changes that happened between the January file and the February file. As of
today the file you are sending has all data for all months every month and when it is sent to the IRS it creates duplicate which in turn create a problem with our ACA reporting.
We have the same issue for the employee dependent file on Column F with the exact same scenario.
Please advise on what it will take to get the programmer to correct this,
