1] EIN
2] Contact Details for Each EIN
3] Total 28 plans, which plan needs to consider as Minimum Essential plan, Self Insured, ACA Plan, Safe Harbor,Plan Enrollment Start Date [If plan is non calendar year started in 2015] ,Is Plan Minimum Value, EE Lowest Cost
4] ACA Parameters - Which salary needs to consider as W2 Wages
5] attached excel OCSD Employee Census.xls has all employee from OCSD production with employee type as full time or Part Time. Let us know if any employee type is not correct in system so that we will correct it. If employee type is blank then employee is considered as Full Time
