We have found that the Primewithoutspace customization Of Cobra plan is not proper. And because of that the Cobra offers are getting offered to all over the Prime.
After discussion with Jennifer F she done the customization properlt.

#1 - All dates should read MM/DD/YYYY - Please update all columns with dates

#2 - COCODE should always be 5 digits - please add the leading zero's

#3 - For column I (JAN2016_EMP (005) file) they do not need NULL listed

#4 - For column R (JAN2016_EMP (005) file) all "0" should be coded to "1"

#5 - For column Q (JAN2016_EMP (005) file) if the plan is Waived, the field should be blank - no NULL's

#6 - For column U (JAN2016_EMP (005) file) - Can you add the header "Invalid" - no data is needed they just need the header

#7 - For column K (JAN2016_EMP (005) file) - They need the date the employee outed out; this should not read NULL - Also please be aware that there is no waive button but a waived plan so you can base this off the waive plans.

It also looks like the plans are not pulling the rates correctly. We need to have the rates based on PT or FT. This would be based on the rates in WT. You can also not include Suburban or Lake Huron to any files.

There are additional items that I will share with you all tomorrow.

We also need to review and possibly update some of the FEIN's - I will have those tomorrow.

1.For column R (JAN2016_EMP (005) file) all "0" should be coded to "1":but actually on file its displaying as '0'.
2.Employee SSN is printed wrongly in Dependent file of Jan-363926854-----II6II926854, Dependent SSN: II84177024, benefit decision date is also displayed as 10/1II/2015.
3.Employee ID is displayed as NULL---We need to display it as blank if its not in system.
4.Dependent suffix id displaying as Null values
5.Similarly here for this employee, "Jill Andrews"-- Workterra system displayed employee is
enrolled in Delete value plan [2015 plan] but on other hand ACA offer plan is not eligible to
employee.

Note: Same errors may be present in Prime with Space file.

1] Employee cost is shown wrong in excel for Amal Sobh employee, This employee is from Class 1 = Garden City HGC and Class 2 = Part Time Employees.
EE Pre Tax cost on excel is 169.94 but it should be zero.

2] EPO 1 Plan (CVHM RNs) is shown in excel for Desert Valley Hospital and Part Time Employees classes but eligibility on plan in workterra is not set for these classes

I just got off the phone with the client and i just want to reiterate what is needed -

Column O -
1. If I am a full time employee (regardless of my tier) the rate should show EE only full time rate
2. If I am a part time employee (regardless of my tier) the rate should show EE only part time rate
3. If I am a part time employee with the "Apply full time rate" flag selected, I should receive the full time EE only rate.

The PT rates are in WT and are pulled based on class 2. Please let me know if you need to know which classes are considered PT.

Jennifer it looks like we are VERY close to having the correct file format except for the one error below.

Can we please fix Column J and then run all the files by month for 2016 I believe Sue sent a list of FEINs by month to make it easier on whom ever runs the reports for Jan-August I have attached again just in case.

Jennifer “I truly appreciate all your hard work” to get this file in the format needed. Now that we have it in the correct format with the correct data I believe it will be a smooth process from here on out.

We are done with analysis and found that there are six employee fall under FEIN# 262887948. 5 out of 6 employees terminated before 01/01/2016 so they did not export on file. GILDA LOPEZ is terminated on March 16 and doesn't have dependent. Hence no dependent for FEIN# 262887948 in January dependent file.Also, we checked GILDA LOPEZ is present in Jan,Feb and March 16 shared file.
As GILDA LOPEZ terminated on march 16 So this employee will not come on April 2016 files onward.

(While doing data analysis we found some employee's has Benefit decision date is
greater than Benefit effective date. So for those employee we have updated benefit
decision date with effective date. Please confirm same point with client.)

We had a weekly ACA meeting this morning and these are the issues at hand regarding the Workterra IRS files:

1) IRS Employee File -We have some employees that do not belong to Prime on the file (see attached) notes in green for each payroll
2) IRS dependent File - Workterra fixed the FEIN number on the IRS employee file but not the IRS Dependent File. Was corrected for October however needs to be corrected for Jan- Sep 2016 and we cannot access the file at FTP URL - https://ftpsecure.ebsbenefits.com/ (see attached)
3) IRS Employee File- have issue with Column I,K & L (see attached) IRS File Specifications and examples sent on email Jan2016 above
Column I - Benefit Effective date -Should only be the first of the month there are other dates in this Column which will not be covered for the entire month due to IRS guidelines.
Column K- Benefit Decision date could/should be the same effective as Colum I.
Column L - Benefits Terminated Date -There are terms dates prior to decision dates, not sure I think the decision date is wrong?
4) Issue with receiving files from Workterra apparently Prime does not have access to this path FTP URL - https://ftpsecure.ebsbenefits.com/ we need all files to go to https://ftpsecure.ebsbenefits.com/thinclient/Login.aspx inbound or outbound

Please advise as we need to start testing our ACA files with the IRS.

Client examples received on 11.16.2016 are all attached.

As per the discussion and latest specification "IRS Files Specs v1.7.0_received 11.16.2016.xls" provided by Amber Love-Jones.

1. Benefit effective date (Column I) always should be 1st of the month.
If employee effective from mid of month then its benefit effective date will 1st of the next month.
( If employee enrolled in Medical plan With Effective date – MM/DD/YYYY – 01/09/2016 or 01/20/2016 then its benefit effective date will be 02/01/2016. )
Please confirm.

2. As per specification for column (L) for Benefits Terminated Date, we are going to keep this column as Blank.

There is also below comment for this column (L) in the specification.
When sending a transaction to denote that the employee is terminating coverage, then provide the date the coverage is effectively terminated (first date of no longer having coverage) in the BenefitsEffectiveDate, and the BenefitsTypeAccepted would be "NO".
If you are sending a transaction to denote that the employer is no longer offering coverage, then provide the date the coverage is effectively terminated (first date of no longer having coverage) in the BenefitsEffectiveDate and the BenefitsTypeOffered and BenefitsTypeAccepted would be "NO".

Do we need to do anything for the above comments?

#1 - We are assuming that benefit effective date will be actual enrollment effective date from system. (If employee is effective from 01/15/2016 then its benefit effective date on file will be 01/15/2016)

#2 - Please update once you get the reply from client. Also share the use case (example) that will help to set logic for Benefits Terminated Date.

#2 Decision Date - This should be the QE date or date they use to initiate the QE not the ACTUAL date that the change was done.

Additional: For the dependent file, it looks like the facility updates that were made to the EE file was not made to the DP file. please make sure the facility lists match -

Once we get confirmation about duplicate employee presents in the Prime WT-6186. Then we will delete these duplicate employee from one of the company and we will provide you final Jan-Nov months files.

It will take almost 5 days to furnish Jan-Nov months files including QA.

File for Dec 2016 will provide in first week of Jan-2017.
